We would love to tell your story.

Each year CHAMP creates Bio Boards which highlight the journey of children and their families impacted by cancer. To submit your child’s story for a Bio Board, and to be included in the Champion Page on the website and other published media for the event, please complete the form below, along with uploading 3-4 high resolution photos.

In Bio section below, please include:

  1. First name of child
  2. Age
  3. Diagnosis
  4. Hobbies
  5. Short story on how cancer has affected you/family
  6. Short sentence of encouragement to the 5K runners/walkers at the event

I give my permission and approve the use of submitted photographs for the creation of my child’s Bio Board along with CHAMP’s fundraising efforts, advertising and publicity, through the CHAMP website: www.septemberchamp.org.
