BIO BOARDS

We would love to tell your story.

Each year CHAMP creates Bio Boards which highlight the journey of children and their families impacted by cancer. The Bio Boards are placed along the course of the 5k and around the booth fair at the event annually. The Bio Boards are a focal point in which the community can get a glimpse into the journeys of a children diagnosed with cancer. CHAMP creates Bio Boards for children on treatment, in remission and also for those who have gained their wings.

To submit your child’s story for a Bio Board, and to be included in the CHAMPion Page on the website and other published media for the event, please complete the form below, and submit with 3-4 high resolution photos.

In the Bio section below, please include:

Full name of child (only first name will be used for the Bio)

Hobbies
Short story on how cancer has affected you/family
Short sentence of encouragement to the 5K runners/walkers at the event or a family motto.

I understand that I can withdraw my consent at any time. Unless I indicate otherwise, there is no time limit on this consent.
